Informed Consent for Gifted Testing

Gifted Testing Colorado provides independent cognitive assessments to help families and schools understand a child's cognitive abilities for the purpose of gifted program consideration. These are educational evaluations only and are not intended as clinical diagnoses.


Your child will complete one or more standardized cognitive assessments, such as the WPPSI-IV (for younger children) or the WISC-V (for school-age children), administered by Kaitlin Smith, EdS, Licensed School Psychologist. Testing typically takes 1.5 to 2.5 hours depending on the child's age and the assessments selected. Breaks are provided as needed. Following the session, a written report summarizing results, observations, and relevant recommendations will be prepared and provided to you.


Participation is entirely voluntary. You may withdraw your child from the assessment at any time. If testing is discontinued before completion, a partial report may or may not be possible depending on how much was completed, and fees may still apply.


Information gathered during the assessment and the resulting report are confidential. They will not be shared with any third party without your written consent, except as required by law or professional ethical standards, for example if there is a concern about the health or safety of your child or others. You may choose to share the report with your child's school or other professionals. Once shared, Gifted Testing Colorado is not responsible for how that information is used or maintained by third parties.


Assessment fees will be discussed and confirmed in writing prior to your appointment. Payment is due at the time of service unless otherwise agreed upon. Please review the cancellation policy in the Terms and Conditions.


Assessment results reflect your child's performance on standardized measures at the time of testing and should be considered alongside other knowledge about your child. As described in our Disclaimer, gifted program placement decisions are made solely by school districts and are not guaranteed by assessment results.

By scheduling services with Gifted Testing Colorado, you confirm that you are the parent or legal guardian of the child being assessed and that you have read and understood the information on this page.

Kaitlin Smith is a mandated reporter under Colorado law (C.R.S. § 19-3-304). This means she is legally required to report to the appropriate authorities if she has reasonable cause to know or suspect that a child has been subjected to abuse or neglect, or is at risk of abuse or neglect. This duty to report exists regardless of any agreement between parties and cannot be waived by parental consent. Reports are made to the Colorado Child Abuse and Neglect Hotline (1-844-CO-4-KIDS). Scheduling services with Gifted Testing Colorado constitutes acknowledgment of this mandatory reporting obligation.


All information shared during the assessment process is kept confidential. However, confidentiality has the following limits: (1) Mandated Reporting — As described above, suspected child abuse or neglect must be reported to authorities. (2) Duty to Warn — If there is a serious, credible, and imminent threat of harm to an identifiable person, disclosure may be required to protect that individual. (3) Court Orders — If a court of law orders disclosure of records, compliance is required. (4) School Disclosures — If you request that assessment results be shared with a school district or educational program, those records may become subject to the Family Educational Rights and Privacy Act (FERPA) once received by the educational institution. (5) With Your Authorization — Information may be shared with other professionals or agencies only with your written consent. Outside of these exceptions, assessment results and all related information will not be shared without your explicit written permission.

Please be aware that email and text messaging are not fully secure or confidential forms of communication. There is some risk that information shared electronically could be intercepted by a third party. By using email or text to communicate with Gifted Testing Colorado, you acknowledge and accept this risk. We recommend that highly sensitive information be communicated by phone or in person. We will make reasonable efforts to protect your information and will not transmit sensitive assessment data via unencrypted email without your consent.


Gifted Testing Colorado retains assessment records in accordance with professional standards and Colorado law. For records involving minor children, records are retained until the child reaches age 25, or for a minimum of seven (7) years following the last date of service, whichever period is longer. This ensures that records remain accessible during the period the child may need them for educational, legal, or personal purposes. After the applicable retention period has expired, records will be securely destroyed. You may request access to or a copy of your child's records at any time by contacting Kaitlin Smith at GiftedTestingCO@gmail.com or 970-231-0734.


Gifted Testing Colorado is a private-pay practice. We do not bill health insurance, accept insurance reimbursement, or submit claims to any insurance company, including Medicaid or Medicare. Because we do not bill insurance, we are not required to comply with HIPAA as a covered entity under federal law. However, we are committed to maintaining the confidentiality and privacy of your child's assessment records consistent with professional ethical standards and Colorado state law. Assessment fees are due at the time of service. A fee schedule and payment options are available upon request. If you plan to seek reimbursement from your insurance provider independently, we can provide you with a receipt or summary of services upon request; however, we cannot guarantee insurance coverage or reimbursement.

These services are intended solely for educational planning, insight into a student’s learning strengths and needs, and for potential gifted identification. Completion of an evaluation does not guarantee that a student will meet the criteria for gifted identification, as results are based on standardized testing and norm-referenced performance. This evaluation is not a medical or psychological diagnosis, nor is it intended to assess or treat mental health conditions. Under current Colorado law, licensed School Psychologists are authorized to provide educational assessments and consultative services, but are not permitted to diagnose or provide therapeutic treatment. Parents are encouraged to share evaluation results with their child’s school or educational team as part of a broader decision-making process.
